
Improving Health and Care Services in our Neighbourhoods
Integrated Neighbourhood Teams
In Brent, we want to work with our communities to improve health and care services in our local areas. As part of this mission, Brent Council, the NHS and our voluntary sector partners are developing "Integrated Neighbourhood Teams". These teams will be made up of health and care workers, volunteers and wider partners who will work together to deliver more localised services that respond to local residents' needs.
By improving co-ordination between services, listening to our communities, and bringing care closer to where you live, we believe we can better help our residents live longer, healthier and happier lives.

The five neighbourhoods
We have divided Brent into five neighbourhoods (or Brent Connects areas) that together cover the whole of the borough. These are:
- Harlesden
- Kilburn
- Kingsbury & Kenton
- Wembley
- Willesden
Each neighbourhood will have a core 'Integrated Neighbourhood Team' that will deliver a seamless service in that area. These teams may look different in different neighbourhoods, as we want them to reflect our local population needs.

We want our neighbourhood teams to:
- Shape our support to what local communities need
- Provide quality, ongoing care closer to where you live
- Deliver care that is centred on the individual and supports all of their health and wellbeing needs
- Develop stronger relationships and meaningful connections between staff (NHS, Brent Council, voluntary sector partners and others), to develop a true partnership approach to the care we provide
We believe we will achieve this through:
- Developing the roles and skills of our staff and promoting greater collaboration and partnership working
- Introducing physical spaces within neighbourhoods where we can deliver a variety of services together in one place
- Improving communication between staff across organisations, and sharing information more effectively to deliver the best possible care
